Adventist Rehabilitation - Shady Grove Medical Center seeks to hire a Physical Therapist for our Outpatient Rehabilitation department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing. As a Lymphedema Physical Therapist, you will perform initial examinations and evaluations to establish an impairment/diagnosis, prognosis, and plan of care prior to intervention for persons served with basic and some complex rehabilitation needs in acute care, outpatient, or inpatient rehabilitation settings. You will provide, or direct and supervise, therapy intervention consistent with the examination results, involving the patient/client and others in the plan of care. The therapist will re-examine the patient/client as necessary to evaluate progress or changes in status and modify the plan of care accordingly or discontinue services. Planning for discharge, considering goal achievement and outcomes, and providing appropriate follow-up or referral are key responsibilities. The physical therapist will discontinue intervention when the patient/client is unable to progress or will no longer benefit from therapy, in coordination with the referring provider/team. Communication, coordination, and documentation of all aspects of patient/client management, including examination, evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in status, reexamination, and discharge/discontinuation of intervention, with members of the care team are essential. You will also supervise and direct patient care activities of assigned rehab aides, office assistants, and volunteers, manage schedules to accommodate census and patient load, attend team meetings, and communicate patient status effectively. Responsibilities include individual and assigned hospital performance improvement and professional development activities, as well as educating and providing consultation to consumers, healthcare professionals, students, and the public regarding the purposes and benefits of therapy. You will mentor and support students during their clinical practices in patient-centered care.
