Patient Rights and Cultural Competency are upheld by respecting religious and linguistic needs, ensuring Advance Directives are addressed, and consistently utilizing translation services for all legal and clinical communications Data Collection is performed with high accuracy by assessing developmental needs, identifying clinical risks such as falls or infections, and maintaining a 95% documentation compliance rate for all patient screenings Planning requires close collaboration with the RN to integrate multidisciplinary assessment findings into a comprehensive, age-appropriate care plan that aligns with hospital policies and identifies specific nursing diagnoses. Implementation involves the safe execution of the nursing plan of care, including hourly rounds, bedside procedures, and strict adherence to security protocols, while prioritizing tasks based on patient acuity Evaluation ensures that all nursing interventions and patient teaching are monitored for effectiveness, accurately documented, and updated each shift to support successful discharge planning and medication reconciliation
