Requirements
- Current Texas LVN License
- Compassionate bedside manner
- Strong communication and organizational skills
- Reliable transportation

- Hospice or home health experience preferred
Responsibilities
- Providing skilled nursing care to hospice patients
- Coordinating with RN Case Managers and interdisciplinary teams
- Monitoring patient comfort and symptom management
- Educating and supporting patients and families
- Maintaining accurate documentation
