This position is for a Licensed Vocational Nurse (LVN) who will provide individualized nursing care to patients and their families under the direct supervision of a Registered Nurse (RN). The role involves collecting basic assessment data, participating in care planning, executing interventions according to the Plan of Care, and contributing to the evaluation of patient outcomes. The LVN is also responsible for communicating with patients and families regarding their Plan of Care and planned treatments. Key functions include detailed assessment data collection, interdisciplinary communication and coordination, thorough documentation, implementation of care plans, informal teaching, patient advocacy, patient and family education, and supporting the evaluation of care plans. New LVNs with less than 12 months of experience will undergo specific orientation and competency training.
