About The Position

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team. Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience. Requirements

  • 2 years recent experience having some contactwith the senior population.
  • Licensed Vocational Nurse: CA, upon hire and Basic Life Support - CPR, upon hire

Responsibilities

  • Receives report from previous shift and organizes and designates shift assignments.
  • Supervises Aides and other Resident Care staff at the direction of the Nurse Manager.
  • Admits, transfers and discharges residents.
  • Makes regular rounds for appropriate patients.
  • Accounts for all residents including Level A residents daily.
  • Takes temperature, pulse, respiration and blood pressure vitals.
  • Monitors residents remaining in units for temporary illnesses such as a cold, flu, etc. Sees that these residents have trays delivered to them for each meal and communicates with residents about expectations.
  • Observes, reports, documents any changes noted with resident to Nurse Manager , physician and immediate family as directed.
  • Answers emergency call signals.
  • Charts in individual Resident Care Chart when appropriate.
  • Responsible for completing 24 hour report.
  • Completes incident report when applicable.
  • Submits charges to manager for any medical supplies given to residents.
  • Carries out physician orders by assisting, performing and documenting procedures such as: specimen collection, irrigations, enemas, heat and cold application, dressing changes (sterile and non-sterile), mechanical comfort, catheterization, postural drainage and oxygen therapy.
  • Assist residents with medication prescriptions and give medications.
  • Re-order medications as needed.
  • Responsible for upkeep of centrally stored medication book.
  • Re-order oxygen as necessary.
  • Provides toileting assistance to residents who regularly or intermittently require it.
