About The Position

Under the supervision of the Registered Nurse Supervisor, the LVN will deliver direct patient care services within the LVN scope of practice required of the nursing component of the LifePlan. They will also provide all day center nursing functions and procedures for the assigned shift, including nursing and personal care services such as toileting, transfers, showers, dressing, grooming, and ADLs. The LVN will promote improved quality of care and/or life, cost-effective medical outcomes, and prevent hospitalizations when possible. They will ensure patients receive appropriate treatments and medications in compliance with the Life Plan and act as a liaison to the patient’s personal physician for any medical concerns. The role involves monitoring and assessing patients in the PACE Day center, assisting with the meal program, providing education and counseling on treatments and medications, and managing medication distribution systems. The LVN will also input patient and prescription data, administer medication, stock and maintain drug inventory, and prepare reports. They will provide one-on-one and telephone support to patients and caregivers, assist in obtaining outside medical services, and maintain open communication with providers and caregivers. Additionally, the LVN will ensure compliance with policies, procedures, quality assurance programs, and safety standards, and maintain working knowledge of all regulations.

Requirements

  • 6 months of experience in direct nursing care of seniors and frail elderly or disabled patients.
  • Graduate of an accredited program of vocational nursing or equivalent.
  • Valid, unrestricted CA Licensed Vocational Nurse.
  • BLS certification.
  • Good oral, written, and interpersonal communication skills.
  • Ability to read, write and comprehend medical information and terminology.
  • Ability to effectively present information, both verbal and written.
  • Demonstrated knowledge of the RN scope of practice.
  • Good interpersonal skills.
  • Skills to provide high-quality care for geriatric patients.
  • Ability to thrive in a team environment.
  • Good organizational and supervisory skills.

  • Must maintain current BLS Certification.
  • Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.

Responsibilities

  • Deliver direct patient care services within the LVN scope of practice.
  • Provide day center nursing functions and procedures for the assigned shift.
  • Provide nursing and personal care services (toileting, transfers, showers, dressing, grooming, and ADLs).

  • Assist in meal program count, ensuring appropriate diets are served.
  • Provide education and counseling to the patient regarding treatment, side effects of medications, and compliance.
  • Establish and maintain a system for medication distribution, patient treatment, medication inventory, biohazard receipt records, and database entry.
  • Generate medication/treatment reports as requested.
  • Coordinate, administer, and monitor patient treatments.
  • Provide training in self-administration of medication to patients.
  • Count, package, pre-package, and label medication for inspection by pharmacist and fill routine orders for non-prescription pharmaceuticals.
  • Input patient and prescription data into the pharmacy computer system.
  • Administer medication according to nationally recognized patient care standards and scope of practice.
  • Assist in the ordering, stocking, and maintenance of drug inventory.
  • Set up weekly medications in med-set.
  • Assist in the preparation of reports and written documentation.
  • Provide one-on-one and/or telephone communication to address issues and provide support to patients and caregivers.
  • Maintain accurate records of all communications and interventions.
  • Provide assistance in obtaining outside medical services.
  • Maintain open communication with medical providers, pharmacist, and caregivers.
  • Maintain compliance to and recommend updates to established policies and procedures.
  • Remind all staff of and ensure compliance to all objectives of the Quality Assurance Program as well as safety, environmental and infection control standards.
  • Maintain working knowledge of all regulations and standards to ensure compliance.
  • Support company and departmental QA/QI initiatives.
  • Maintain and submit orders to maintain an adequate inventory of forms, clerical and pharmaceutical supplies, paper, and equipment.
  • Follow established departmental policies and procedures, objectives quality assurance program, safety, environmental and infection control standards.
  • Enhance professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Attend meetings, including PACE morning meetings and IDTs, as needed, or required and participate on committees as directed.
  • Perform other related duties as assigned or requested.
