About The Position

We are seeking a compassionate and highly organized Licensed Vocational Nurse (LVN) to join our care team. In this role, you will be a key liaison between patients, providers and the Program Director, ensuring a seamless care experience from initial contact through ongoing follow-up. You’ll work closely with providers, specialists, and our internal team to deliver personalized support that goes beyond traditional care models.

Requirements

  • Active LVN license in California required.
  • Minimum 1-2 years of clinical experience; experience in oncology or imaging preferred.
  • Strong understanding of EMR platforms; Athena health experience highly preferred.
  • Excellent communication, multitasking, and problem-solving skills.
  • Comfortable working across multiple digital platforms in a fast-paced, team-oriented environment.
  • Ability to manage sensitive patient interactions with professionalism and empathy.
  • Organized, tech-savvy, and driven by a commitment to exceptional patient care.
  • Must be IV Certified with IV experience.

Nice To Haves

  • experience in oncology or imaging preferred

Responsibilities

  • Provide superior customer service to all patients through in-person visits, emails, phone calls, text messaging, and telehealth communication.
  • Perform LVN clinical tasks, including taking vitals, administering IVs, collecting patient specimens, administering medications and vaccinations, and conducting basic assessments.
  • Coordinate and manage day-to-day patient care activities, including care plan implementation.
  • Attend offsite medical appointments to provide patient care.
  • Oversee patient intake and registration, ensuring all patients are fully checked in before their appointments.
  • Support EMR workflows.
  • Triage and respond to customer service inquiries across various online platforms.
  • Monitor and respond in real time to communications from organization devices and platforms.
  • Support various customer service team projects as delegated by the Program Director.
  • Collaborate with the care team to enhance patient engagement and satisfaction.
