About The Position

Angels of Care Pediatric Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering patient-centered care according to physician orders and active plans of care, ensuring positive outcomes through ongoing assessments, medication administration, treatment, and family education. Requirements

Active RN or LPN/LVN license

Responsibilities

Provide direct, one-on-one medical care to infants, children, and/or adolescents in their home.
Focus on the medical needs and treatment of patients according to the active plan of care, as given by the physician and other appropriate personnel.
Conduct on-going patient care and assessments.
Administer prescribed medication, treatments, and therapies.
Coordinate care.
Educate family members on patient clinical care to enhance positive outcomes.
Implement preventative initiatives to protect the quality of care for patients.
