Under the Direction of the Nurse Manager, and guided by the policies and procedures of Ambulatory Services, the Licensed Vocational Nurse (LVN), provides care to patients in the UCSF Department of Urology and UCSF Department of Women’s Health. Patients in this clinic receive minimally Invasive Gynecologic Surgery, Urogynecology, and Urology services at our Berkeley Outpatient Center. The LVN will play a critical role in the patient experience including optimizing clinic flow, providing outreach to patients, and working collaboratively with the team to create a patient centered environment. In collaboration with the physician and nursing staff, the LVN has the clinical responsibility for maintaining a safe environment for patients and staff, and for daily operations. The LVN is a team player who works closely with others and who is flexible in dealing with the changing priorities. They are a self-reliant individual who synthesizes their knowledge of practice operations to problem-solve, prioritize, and facilitate complex transactions during their daily activities. They are responsible for administering medications, assisting with a variety of procedures, practicing basic assessment of patient medical and psycho-social needs, participating in patient education, and assisting medical providers in a variety of ways. All components of nursing care will be individualized to meet the patient’s physiological and psychosocial developmental level and needs. The Licensed Vocational Nurse has excellent oral and written communication skills; can work well with minimal supervision in a complex, fast pace and challenging environment. They are always sensitive to the needs of patients, staff, and providers. This position makes a difference for patients in an outpatient care unit by providing excellent nursing care and customer service, facilitating and ensuring the accuracy of the information flow between medical, hospital staff and departments to maximize unit efficiency. They consistently project a professional and courteous demeanor as well as demonstrating UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions. The LVN will utilize computer skills in ApeX and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. S/he is required to adhere to the Telephone and House Standards as adopted by the Medical Center.

Requirements

• 2 year experience as an LVN in an ambulatory clinic within the last 3 years
• 1 year ambulatory experience within the last 2 years in at least one of the following specialties: OBGYN, UROLOGY, UROGYNECOLOGY
• Experience administering injections
• Knowledge and abilities essential to the successful performance of the duties assigned to the position.
• Excellent communication skills.
• Experience working in a fast-paced clinic setting
• Licensed as a Vocational Nurse in the State of California
• Current American Heart Association BLS Certificate

Responsibilities

• Identifies the needs and responses of patients to medical regimen and treatment through observation of symptoms, emotional state, cultural and socioeconomic background. Able to collect, process, and analyze data. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patient to appropriate members of the health care team. Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g. obstetrical emergencies, chest pain, shortness of breath. Reports emergency or unsafe patient care conditions on the Incident Reporting System and to the management team.
• Works cooperatively and participates with the health care team to discuss patient’s alternatives to health care services and to develop short and long range goals to meet the patient’s needs. Plans as appropriate for the needs of patient using knowledge of physical and emotional processes as well as cultural and socioeconomic backgrounds. Identifies appropriate resources (e.g. family, significant others, monetary, community organizations) in planning for patient care.
• Provides individualized nursing care which responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient. Sets up and maintains cleanliness of exam rooms including periodic exam room disinfection and organization and removal of expired supplies. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments. Assists in complex medical procedures such as office Manual Uterine Aspiration, hysteroscopy, cystoscopy, and urodynamics testing. Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC on CLIA waived tests and reports any tests out of QA range to supervisor. Uses appliances and equipment correctly and maintains safe practice. Evaluates and documents patients’ responses to nursing care in the electronic medical record, including pre/post procedures and treatments. Uses knowledge of cognitive, physical, emotional and chronological maturation process in evaluating care for all ages of patients. Practices basic assessment and applies communication skills for the purpose of patient care and education. Administers and documents the administration of medication while maintaining current knowledge of pharmacology and competency in modes of administration. Documents telephone messages and chart notes in a thorough and grammatically correct manner. Responds appropriately in emergencies. Is knowledgeable about the contents of the code blue cart and/or the emergency kits, and can initiate and assist in CPR. Understands the procedures for medical emergencies, code blue and other emergency/disaster situations. Participates in safety drills. Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice. Complies with activities mandated by JCAHO/Title 22/NCQA. Completes annual competency testing as specified by Ambulatory and the individual practice.

Adjusts clinical support activities to reflect changes in priorities during practice sessions. Maintains open communication and facilitates continuity of care for patients/and or family with members of the health care team through phone contact, verbal interaction. Maintains accurate, records and documentation which include patient complaints, nursing observations, interventions and care plan, nursing actions/teaching performed and patient response to the action. Obtains, records, and conveys information, clearly and concisely. Demonstrates courtesy, common sense, and follow-through in all telephone/electronic communications, maintaining the Telephone Standards as established by the Medical Center. Completes all tasks within assigned InBasket to include cross-coverage of InBaskets of other care team and other practices across Obstetrics & Gynecology. Fosters/maintains positive public relations (patients, family, visitors and staff). • Greets and welcomes patients making eye contact and utilizing AIDET standards. Registration, check-in, and check-out of patients Determines a patient’s co-pay obligation and collects it at the time of the visit; may also collect payments on patient accounts. Coordinates appointments with multiple providers as required, including new and established patient appointments using Apex and its related components. Schedules procedures and tests providing appropriate instructions to patients. Discusses practice policies and procedures with patients and referring physicians. Answers questions about provider schedules and acts as a resource to other medical center practices and ancillary service administrative staff. Adheres to the provider productivity standards as established in APEX templates for the practice and schedules appointments with the appropriate appointment type. Triages all telephone calls, screening for emergencies and routing all messages, requests for same day appointments, refill requests, etc. to the appropriate box. Covers Apex in-baskets and phone messages when other members of the team are absent. Demonstrates an ability to adjust priorities as required for smooth operation of the practice and notifies the clinical staff. Provides administrative support to providers in coordination of patient care (i.e. sending patient letters, educational materials, results of lab tests, etc.) Assists in maintaining current filing and scanning.
