About The Position

Pine Creek Care Center is seeking a compassionate, dependable, and clinically skilled Licensed Vocational Nurse (LVN) to join our full-time NOC Shift team. If you're passionate about providing exceptional care, leading by example, and creating a safe, comfortable environment for residents overnight, we'd love to meet you. As an LVN, you'll play a vital role in delivering quality nursing care, supervising CNAs, administering medications and treatments, collaborating with physicians and interdisciplinary teams, and ensuring our residents receive the highest level of compassionate care.

Requirements

Current, active, and unencumbered California LVN license
Graduate of an accredited LVN/LPN program
Current CPR certification
Strong leadership, communication, and critical thinking skills
Ability to work independently while collaborating with an interdisciplinary team
Passion for serving seniors with dignity, compassion, and respect

Nice To Haves

Skilled nursing or long-term care experience preferred
Experience with PointClickCare (PCC) preferred

Responsibilities

Provide direct nursing care to skilled nursing residents.
Administer medications and treatments safely and accurately.
Supervise and support Certified Nursing Assistants (CNAs).
Monitor resident conditions and promptly communicate changes to physicians and families.
Complete accurate and timely documentation in accordance with state and federal regulations.
Admit, transfer, and discharge residents as needed.
Ensure individualized care plans are followed and updated.
Participate in quality assurance and infection prevention initiatives.
Maintain compliance with all nursing policies, safety standards, and regulatory requirements.
Foster a warm, respectful, and resident-centered environment.
