Napa Post Acute is looking for a knowledgeable and detail-oriented LVN MDS Coordinator to join our dedicated team. Requirements

  • Current California LVN license
  • MDS experience preferred (or strong clinical background with willingness to learn)
  • Detail-oriented with strong organizational skills
  • Ability to work independently while being a team player

Nice To Haves

  • Knowledge of PDPM and regulatory requirements is a plus

Responsibilities

  • Coordinate and complete MDS assessments in accordance with federal and state regulations
  • Ensure accurate and timely submissions
  • Collaborate with interdisciplinary team members to support care planning
  • Monitor documentation to ensure compliance and optimize reimbursement
  • Support clinical staff with MDS-related processes and education
