The LVN / LPT assists with medication support services provided by the psychiatrist, nurse practitioner, or physician’s assistant. Medication support services may include but are not limited to; med support appointments, taking vitals, medication education, medication management support and/or administration, including injections. Follow up communication, tracking and documentation for labs, prescriptions and appointments. Provides direct client care including medications and basic medical intervention and education, following standard nursing practices. This role is part of the Full Service Partnership - TAY - Sacramento program, a community-based outpatient program serving Transitional Age Youth (TAY) ages 16-25, offering Peer Support & Independent Living Skills Development, Intensive Case Management Support, Individual/Family Counseling, and Mental Health & Medication Management Support.
