The LVN / LPT provides direct client care including medications and basic medical intervention and education, following standard nursing practices. This is a 24/7 voluntary residential setting specifically for Transitional Age Youth (TAY) at risk and in need of a higher level of care, but lower than a psychiatric hospital. The facility is equipped to serve up to 15 Youth. It provides mental health services that include individual therapy, group rehabilitation, medication management, and case management. The program incorporates the tenets of recovery, peer support, and identifying community connections. It also connects individuals to educational and vocational services and initiates the process of locating housing.
