About The Position

Tryfacta is seeking a LVN / LPN for our client in Rochester, NY 14621-3011. This is a 13 Week contract assignment. The role involves providing direct nursing care to adult and geriatric medical-surgical/telemetry patients under RN support. Responsibilities include assessing and monitoring patients' conditions, administering medications, monitoring telemetry, providing wound care, and managing various medical-surgical needs. The position requires adherence to isolation precautions, PPE protocols, and safety procedures. Effective communication with patients, families, and the healthcare team is essential, as is accurate documentation in Epic EMR. The nurse will collaborate with an interdisciplinary staff and manage an assigned patient load. Floating between Med-Surg units may be required. The role involves working 3x12-hour shifts, including every other weekend and required holidays.

Requirements

• Active New York LPN/LVN license — pending licenses are not accepted.
• Minimum 1 year of LPN/LVN experience in an acute-care setting.
• Recent acute-care experience within the last 2 years.
• Previous experience independently managing a patient assignment with RN support.
• Epic EMR experience is required.
• BLS certification is required.
• Experience with medication administration, including oral, IM, SQ, NG/G-tube, ear, and eye medications.
• Experience with telemetry monitoring, cardiac monitoring, and 12-lead EKGs.
• Ability to perform vital signs, blood glucose monitoring, pain assessment, and intake/output monitoring.
• Knowledge of wound care, simple dressing changes, drains, Foley catheter care, and feeding tube management.
• Ability to follow isolation precautions, PPE, restraint, and patient-safety protocols.
• Strong communication skills and ability to promptly report patient status changes to the RN/MD.
• Willingness to float between the 5400 and 5500 Med-Surg units within the LPN/LVN skill set.
• Willingness to work 3×12-hour shifts, every other weekend, and required holidays.
• Travel nursing experience is required.

Nice To Haves

• Medical-Surgical/Telemetry experience preferred and ability to care for adult and geriatric patients.
• Experience with IV pumps and IV therapy; ability to start/maintain IVs preferred.

Responsibilities

• Provide direct nursing care to adult and geriatric medical-surgical/telemetry patients under RN support.
• Assess and monitor patients' conditions, including vital signs, pain, neurological status, intake/output, and blood glucose levels.
• Administer medications through oral, IM, SQ, NG/G-tube, ear, and eye routes as ordered.
• Monitor patients on telemetry, recognize changes in cardiac rhythms, and obtain 12-lead EKGs when required.
• Provide and monitor oxygen therapy via nasal cannula and assist with respiratory care.
• Perform and assist with wound care, simple dressing changes, JP/Hemovac drains, and pressure-ulcer prevention.
• Provide Foley catheter insertion and care and assist with clean-catch urine collection.
• Provide care for patients with feeding tubes, colostomies/ileostomies, and other medical-surgical needs.
• Follow isolation precautions, PPE protocols, and safety procedures, including management of physical restraints when required.
• Communicate changes in patients' conditions promptly to the RN and/or physician.
• Collect, label, and handle patient specimens according to hospital protocols.
• Maintain accurate and timely documentation in Epic EMR.
• Respond appropriately to cardiac and medical emergencies and collaborate with the Rapid Response Team when necessary.
• Work collaboratively with RNs, physicians, respiratory therapists, physical therapists, pharmacy, IV teams, phlebotomy, social services, and other interdisciplinary staff.
• Manage an assigned patient load of approximately 1:5–6 on days and 1:6 on nights/weekends.
• Float between the 5400 and 5500 Med-Surg units as needed within the LPN/LVN scope and skill set.
• Maintain effective communication with patients, families, and the healthcare team.
• Follow hospital policies, infection-control standards, and patient-safety protocols.
• Work 3×12-hour shifts, including every other weekend and required holidays as scheduled.
