About The Position

Aspire Home Healthcare is seeking compassionate Licensed Practical Nurses (LPNs) to join their growing Dublin, Ohio team. As an LPN in home health, you will provide direct patient care, monitor progress, and ensure the highest quality outcomes while working independently with the support of an experienced clinical leadership team. Requirements

Current LPN license in the state of Ohio
Physical exam within 12 months prior to first patient contact
Current TB test (within 12 months) and CPR certification
Valid driver’s license and proof of auto insurance
Ability to attend a 2-day orientation at our Dublin office

Responsibilities

Obtain and document patient medical history and current condition
Conduct brief physical assessments (vitals, ambulatory status, mental status, etc.)
Implement physician-ordered care plans and nursing interventions
Educate patients and families about care routines and health management
Provide skilled nursing care and maintain confidentiality per HIPAA regulations
Evaluate the effectiveness of care and update the plan of care as needed
Submit all documentation and visit notes in a timely manner
Collaborate with the interdisciplinary care team to ensure excellent outcomes
