About The Position
Tryfacta is seeking a LVN / LPN- LTC for our client in Vestal, NY 13850. This is a 16-week contract assignment. The facility is a State/Government Run Long-Term Care Center, offering rehabilitation therapies, skilled nursing, memory care, wound care, and more. The role involves administering medications and treatments, performing medication passes, monitoring resident conditions, completing documentation, providing nursing care, and supervising CNAs.
Requirements
- Active license (state)
- Minimum of 1 year of Long-Term Care (LTC) experience required
- Must be able to pass a medication administration (med pass) test and clinical observation
- Strong attendance and punctuality required
- Organized and detail-oriented
- Excellent communication skills
- Personable and professional demeanor
- Ability to work with individuals from diverse cultures and personalities
- Team player with a positive attitude
- Must be dependable with consistent attendance
- Ability to work in a fast-paced environment
- Must be flexible with scheduling needs
Nice To Haves
- PCC (Point Click Care) charting experience is not required for submission.
Responsibilities
- Administer medications and treatments as prescribed
- Perform medication passes accurately and safely
- Monitor and report changes in resident condition
- Complete timely and accurate documentation
- Provide nursing care in accordance with facility policies
- Supervise CNAs on assigned units
