This role requires genuine curiosity about AI, comfort experimenting with tools like Claude, ChatGPT, and workflow automation platforms to support clinical documentation and care coordination tasks, and the judgement to know when AI helps and when it doesn't. If AI makes you uncomfortable, this is not the right role. We are seeking a motivated and detail-oriented LVN/LPN to join our Chronic Care Management team. In this role, you will play a critical role in patient care by conducting comprehensive assessment calls, developing and educating patients on care plans, performing Behavioral Health Integration (BHI) assessments, and assisting patients with medical device troubleshooting.

Requirements

  • Active LPN/LVN Compact License required
  • Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must.
  • Strong communication and organizational skills
  • Proficient with computers, EMRs, and telehealth tools
  • Minimum fiber internet speed of 50 Mbps download / 10 Mbps upload
  • Hardwired internet connection required
  • Private, HIPAA-compliant workspace

Nice To Haves

  • 1–2 years of clinical experience in a GI or gastroenterology setting — clinic, hospital, ambulatory surgery center, or virtual/telephonic care environment
  • Experience triaging high-volume GI-related patient calls including bowel prep questions, post-procedure concerns, flare management, and symptom escalation
  • Familiarity with common GI conditions including GERD, IBS, IBD, Crohn's disease, ulcerative colitis, diverticulitis, and colorectal cancer screening protocols
  • Knowledge of GI-specific medications, bowel prep protocols, and pre/post procedure patient education
  • Ability to de-escalate distressed or difficult GI patients telephonically with empathy and clinical confidence

Responsibilities

  • Conduct thorough assessment calls with patients to gather relevant information about their health status, concerns, and needs
  • Identify and escalate patient needs, including refill requests, appointment scheduling, and connecting patients with community resources
  • Develop comprehensive care plans based on patient assessments
  • Perform Behavior Health Integration (BHI) assessments to evaluate and address the mental health aspects of patient care
  • Maintain compliance with company policies and applicable regulations
  • Perform other duties as assigned
