Requirements
- Completion of a Licensed Vocational Nursing Education Program
- Demonstrated ability to work with health team personnel
- Minimum 1 year experience in acute care facility or community health agency
- Current LVN license
- Current CPR certification
- Recent physical
- TB, Tdap, MMR, Varicella, HepB immunizations
- Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs)
- Ability to learn and assimilate the NetSmart Computer Program/System and navigate through multiple technical interface portals to enter and retrieve information related to patient care
- Available Tuesday through Saturday, 9am to 5pm

Responsibilities
- Taking all hospice information calls regarding the intake of new referrals to facilitate an evaluation for hospice service
- Managing all patient intake functions throughout the entire intake process
- Providing Hospice evaluations as directed
