About The Position

Hinds Lifecare, a well-known and respected nonprofit agency in California’s Central Valley and a pioneer in hospice and palliative care, is seeking an LVN Intake Coordinator to join their team. Requirements

  • Completion of a Licensed Vocational Nursing Education Program
  • Demonstrated ability to work with health team personnel
  • Minimum 1 year experience in acute care facility or community health agency
  • Current LVN license
  • Current CPR certification
  • Recent physical
  • TB, Tdap, MMR, Varicella, HepB immunizations
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs)
  • Ability to learn and assimilate the NetSmart Computer Program/System and navigate through multiple technical interface portals to enter and retrieve information related to patient care
  • Available Tuesday through Saturday, 9am to 5pm

Responsibilities

  • Taking all hospice information calls regarding the intake of new referrals to facilitate an evaluation for hospice service
  • Managing all patient intake functions throughout the entire intake process
  • Providing Hospice evaluations as directed
