About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program for individuals 55 years or older, living with chronic illness or disabilities, who need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. This position travels to participant's home and involves extensive use of reliable transportation throughout San Diego County for home visits.

Requirements

• Graduate of a school of Licensed Vocational nursing.
• Current California Licensed Vocational Nurse license, CPR and first aid certification required.
• Shall be at least 18 years of age.
• A minimum of one year’s experience in working with the frail or elderly population required.
• Shall have either training or related experience in the job assigned.
• Must be able to clear fingerprints through DOJ and FBI in addition to a drug screen clearance.

Responsibilities

• Provides skilled nursing services (medication administration, wound care, etc.) to the participants of St. Paul’s PACE in their homes.
• Maintains the participants’ medical record with thorough documentation.
• Communicates participants’ changes and progress to appropriate staff.
