LVN - Home Health (Field) Full-Time
About The Position
We are seeking a Licensed Vocational/Practical Nurse (LVN/LPN) who is passionate about providing hands-on patient care in their homes. This role offers a flexible schedule, the opportunity to provide 1:1 care, and a supportive team environment. Unlike large systems with high patient ratios and constant rushing, we focus on quality care and professional development. This role requires local travel, working in patients' homes, occasional on-call duties for emergency calls and visits, and timely, accurate documentation.
Requirements
- Graduate of an accredited LVN/LPN program.
- Active TX LVN/LPN license.
- 1+ year clinical experience required.
- CPR certification and TB testing.
- Strong nursing and interpersonal skills.
- Reliable car, license, and insurance required for local travel.
Nice To Haves
- Home health experience preferred.
- Bilingual (Spanish).
- IV certified.
- Experience with OASIS or Axxess EMR.
Responsibilities
- Provide 1:1 care in patients' homes.
- Perform timely and accurate documentation.
- Pitch in for occasional emergency calls and visits while on-call.
