About The Position

The primary function is to administer skilled nursing care and care coordination for patients of all ages. Liason for the field and office for the hospice and home health interdisciplinary team, patient/family and agency.

Requirements

Graduate of an accredited school of vocational nursing.
Current State license as a Licensed Vocational Nurse.
Current State Driver's License.
Two years experience as a Licensed Vocational Nurse in a clinical care setting.
Reliable transportation.
Valid and current auto liability insurance.
Case management required.
Texas LVN License (Required).

Nice To Haves

Hospice or home health experience preferred.
Spanish speaker preferred but not required.
Wound care, infusion, vent and trach experience preferred.
Experience working with all ages preferred.

Responsibilities

Administer skilled nursing care and care coordination for patients of all ages.
Serve as a liaison between the field and office for the hospice and home health interdisciplinary team, patient/family, and agency.
Under the direction of the RN, supplement the nursing care needs of the patient as provided by the RN and as specified in the POC.
Assist the RN in carrying out the plan of care.
Assist the patient in learning appropriate self-care techniques.
Provide services in accordance with organizational policies and procedures and regulations that define scope of practice.
Assist the RN and/or physician in performing specialized duties related to end-of-life care.
Prepare equipment and materials for treatment adhering to aseptic technique as required.
Assess patient/family response to care, ensuring communication of information to appropriate team members.
Promote Agency philosophy and administrative policies to ensure quality of care.
Perform QA and patient coordination in the office.
Conduct follow-up visits in the Katy/Houston area.
