About The Position

The Licensed Vocational Nurse (L.V.N.) delivers patient care within the L.V.N. scope of practice to patients of all ages, including geriatric, adult, adolescent, pediatric, and newborn. This role also supports facility staff, physicians, visitors, vendors, and the general public. The LVN must be able to obtain and interpret patient information and understand the range of treatment needed for all patient groups. The position requires flexibility to work scheduled hours, including holidays, call-backs, weeknights, weekends, and on-call duties. The LVN agrees to participate in emergencies and community disasters as required by the California Emergency Services Act. This role also involves an affirmative duty to report perceived misconduct and violations of laws, regulations, policies, or standards. The employee must work well under pressure, meet deadlines, and demonstrate cooperative behavior.

Requirements

  • Minimum of least two (2) years experience as a LVN in subacute and acute care setting preferred.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence if required.
  • Ability to calculate figures as it pertains to medication administration if required.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Basic computer skills.
  • Current licensure as a Licensed Vocational Nurse in the state of California.
  • Current BLS certification.
  • Crisis Prevention Intervention (CPI) certification or obtain within one (1) year of assignment to department.
  • IV and blood withdrawal Certification or obtain within 1 year of hire date.

Responsibilities

  • Demonstrates support of the philosophy, policies and goals of Patient Care Services and the individual unit.
  • Documents care on the patient care record, accurately, consistently and in a timely manner.
  • Prioritizes work activities to achieve optimal patient care.
  • Initiates nursing orders to ensure safety of patients at risk.
  • Completes incident reports; reports incidents to Director, Clinical Manager, or nurse.
  • Ensures availability of supplies and equipment for specific patient needs.
  • Compliance with all regulatory bodies: (DHS).
  • Provides staffing support to the unit as requested by Director, i.e. scheduling flexibility, working extra shifts on short notice, low census days.
  • Demonstrates ability, flexibility to assist in unusual clinical situations, i.e. temporary reassignment, changes in patient acuity/reassignment, unscheduled admission, and life-threatening patient situations.
  • Provides clinical support to reassigned personnel and prn personnel within L.V.N. practice.
  • Communicates essential information to Director, co-assigned nurse and/or Registered Nurse, including significant changes in patient status and feedback.
  • Works within their scope of practice as defined by the California Board of Vocational Nurses and Psychiatric Technicians.
  • Possesses assessment skills to include respiratory status.
  • Administers medication to residents in a safe and competent manner and maintains a medication error rate less than 5%.
  • Practices appropriate infection control techniques.
  • Delegates appropriate tasks to unlicensed personnel and ensures their completion.
  • Interpersonal relationships demonstrate an understanding of personal responsibility to represent nursing and Sierra View Medical Center in a courteous, helpful and productive manner.
  • Participates in quality assurance activities and integrates findings into practice.
  • Demonstrates sensitivity and awareness of patient's rights in handling confidential patient information.
  • Adheres to hospital standards, policies and procedures relative to situation, including fulfilling work schedule commitment, dressing appropriately, and attending unit meetings.
  • Available and functions as a resource to other caregivers, sharing expertise, demonstrating teamwork, and fostering integration of new personnel.
  • Demonstrates adherence to the “National Patient Safety Goals for Hospitals” as defined by scope of practice, including documentation, waived testing, knowledge of role, and patient engagement.
  • Delivers patient care with compassion, empathy and respect utilizing the nursing process model.
  • Assists in obtaining general patient information.
  • Encourages patient, family, significant other participation in identification of needs, i.e. physical, emotional, educational, discharge, etc.
  • Under direct supervision of the R.N., identifies pertinent patients' needs/problems.
  • Is responsible for the treatment of care appropriate to the ages of the patients served, i.e. Adolescent, Adult, and Geriatric. Demonstrates the ability to obtain and interpret information in terms of the patient's needs; possesses knowledge of growth and development; and has an understanding of the range of treatment needed for these patient groups.
  • Under direct supervision of the R.N. Participates in ongoing revision of individualized patient care plan identifying measurable long/short-term goals and discharge plans.
  • Initiates physician's verbal/written ordered services.
  • Attempts to ensure patient understanding of special preparation for procedures performed in and by other departments to facilitate a coordinated effort toward optimal patient care.
  • Delivers basic nursing care utilizing policies, procedures and standards of care.
  • Accurately reports and records assessments, patient data, medication, treatment, procedures and contributes to evaluation of progress towards goals.
  • Evaluates patient response to specific nursing interventions.
  • Assumes responsibility for continued professional growth through participation in continuing education opportunities.
  • Attends unit, hospital and other available in-service programs according to education policy.
