About The Position

The LVN Case Manager works with new residents to assess their conditions and requirements, developing customized care plans to meet their specific personal needs. Regularly evaluating and updating these plans is essential. They also provide education to residents and their families on following health plans. This role controls and administers managed care cases for plan members admitted to provider facilities or utilizing services outside these facilities. Responsibilities include checking prior authorizations, performing concurrent reviews, indicating appropriate levels of care and utilization of services, developing discharge plans, and ensuring quality and cost-effectiveness. The LVN Case Manager develops a plan of care from admission to discharge and may coordinate assignments with care partners, assisting in supervising team members accountable for resident care and satisfaction.

Requirements

  • Completion of Accredited Licensed Nursing Program (LVN) and current state license
  • Minimum of 2 to 4 years professional managerial experience in related field
  • 2 years case management experience
  • Clinical observation and assessment skills
  • Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members
  • Current LVN license
  • Current CPR certification

Responsibilities

  • Works with new residents to assess their conditions and requirements along with the purpose of developing care plans customized to meet the specific personal needs of residents
  • Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals and interventions
  • Coordinate the implementation of the plan of care and insure care coordination and case management for each patient. As applicable, apply specific criteria for admission and re-certification to establish appropriate levels of care and patient’s eligibility
  • Assesses resident’s needs at the time of move in and continually evaluates residents’ condition. Promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community’s safety procedures)
