The LTSS Service Coordinator RN Clinician is responsible for the overall management of a member's case within the scope of their RN licensure. This role involves supervising and directing non-RN clinicians in accordance with state law and contract, as well as developing, monitoring, evaluating, and revising the member's care plan to meet their needs. The primary goal is to optimize the member's healthcare across the care continuum. This position requires performing face-to-face clinical assessments to identify, evaluate, coordinate, and manage member needs, including physical health, behavioral health, social services, and long-term services and supports. This is a field-based role that requires travel up to 1 hour away daily and may involve occasional office attendance. Candidates must reside in specific Texas counties: Lubbock, Hale, Wilbarger, Potter, Randall, and Wichita. Alternate locations may be considered if candidates are within commuting distance of an office, but those not within a reasonable commuting distance will not be considered unless an accommodation is granted.
