The MyCare Ohio health plan aims to deliver high-quality, trauma-informed, culturally competent, person-centered coordination for all members, addressing physical health, behavioral health, long-term services and supports, and psychosocial needs. This field-based role allows associates to primarily work in the field, traveling to client sites or designated locations, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement. The LTSS Service Coordinator-Clinician is responsible for the member's case. As required by applicable state law and contract, the Clinician contributes to the LTSS care coordination process by performing activities within the scope of licensure, including telephonic or face-to-face assessments for the identification, evaluation, coordination, and management of member's needs, encompassing physical health, behavioral health, social services, and long-term services and supports.
