The LTSS Service Coordinator Clinician works under the direction and supervision of an RN, who has overall responsibility for the member’s case as required by applicable state law and contract. Within the scope of licensure, the clinician contributes to LTSS care coordination by assisting the responsible RN with telephonic or face-to-face assessments and helping identify, evaluate, coordinate, and manage member needs related to physical health, behavioral health, social services, and long-term services and supports. This is a field-based position involving travel to client sites or designated locations, with occasional office attendance for meetings or training. Candidates should reside in Richmond City, Henrico, Hanover, Essex, King William, Petersburg, or Westmoreland, Virginia, or within commuting distance of an office. The clinician provides collected information to the RN, who verifies and interprets it, conducts further assessments as needed, and develops, monitors, evaluates, and revises the member’s care plan. Clinical decision-making is guided by member needs, disease processes, terminology, and clinical guidelines, and does not require nursing judgment.
