The LTSS Service Coordinator- Clinician is responsible for working under the direction/supervision of an RN or a Licensed Social Worker, with overall responsibility for the member's case, as required by applicable state law and contract. This role contributes to the LTSS care coordination process by performing activities within the scope of licensure, including assisting the responsible RN or LSW with telephonic or face-to-face assessments for the identification, evaluation, coordination, and management of the member's needs, including physical health, behavioral health, social services, and long-term services and supports. This is a field-based role that enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement. Candidates must live in Cass or Northern Miami County.
