Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs. This is a field-based position, and applicants must reside in Sedgwick, Sumner, or Harvey County, Kansas. The ideal candidate should have case management experience, be computer savvy, and be proficient in Microsoft applications. Strong communication and customer service skills are essential, along with excellent attention to detail, assessment skills, organizational and management skills, and effective time management. The ideal candidate should also be a self-starter who can work independently, take initiative, and manage responsibilities effectively. The standard work schedule is Monday through Friday, from 8:00 AM to 5:00 PM.
