Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs. This is a field-based position, and applicants must reside in Seminole County, Florida. The ideal candidate will have experience in long-term care, community-based resources and referrals, assessments, and care planning. Candidates should be computer savvy and possess excellent communication and customer service skills, be team-oriented, and demonstrate strong problem-solving skills. Bilingual Spanish skills are preferred.
