About The Position

This position provides program support to the Medicaid Case Management, Care Coordination, and Health Home programs. Responsibilities include completing approved Program Aide Work Requests, eligibility checks, mailings, database entry, report maintenance and reconciliation, file management, processing program referrals, managing transitional care notifications, and communicating with clients and community-based organizations both over the phone and in person. This position also provides coverage for receptionist breaks and lunches.

Requirements

  • Two years of college level courses in a relevant field and at least two years of experience providing direct human services as well as administrative experience.
  • Possession of a valid driver’s license and minimum state-required vehicle insurance and have use of reliable transportation.
  • Ability to work synergistically as a team member with a broad variety of diverse staff, clients and providers to establish and maintain effective working relationships.
  • Ability to communicate verbally in the English language in face-to-face one-on-one settings, group settings, by personal computer or through use of telephones.
  • Ability to produce written documents with clearly organized thoughts using proper English sentence construction, punctuation, and grammar.
  • Computer and software skills; ability to operate general office equipment; work at a desk using phone and computer for up to eight hours a day.
  • Specific knowledge of Excel, Work, Outlook, Access or the ability to become efficient in these programs.
  • Knowledge of general office practices and procedures, ability to work independently and prioritize workload.

Responsibilities

  • Provides program and administrative support using software and computer applications to include WORD, EXCEL, ACCESS, PowerPoint.
  • Maintains and updates program resource and referral data systems.
  • Performs general copying and facsimile functions, collating, and packet production, utilizing word processing system for correspondence, labels, mailing lists, resource inventory, etc.
  • Manages and performs special projects at the direction of the LTSS Supervisor or LTSS Program Manager.
  • Responds to work requests for acquisition of approved equipment and environmental modifications.
  • Authorizes payment for services and equipment rendered by contractors.
  • Responsible for completing Program Aide Work Requests to support the functions of the Medicaid Case Management, Care Coordination Organization (CCO), and Health Home Lead programs.
  • Maintains accurate and up-to-date records in multiple databases.
  • Collaborates with other AAADSW staff to produce flyers, organize and distribute monthly mailings.
  • Confirm monthly program eligibility.
  • Assists with processing and assigning new files/referrals.
  • ACCESS Database maintenance includes uploads of new referrals, completing reports and queries and completing regular updates to electronic records.
  • Process and Reconcile EXCEL billing spreadsheets, process updates, format as needed and complete reports.
  • Ability to work within multiple complex web-based programs and internal document systems and complete assigned projects by identified deadlines.
  • Provides coverage for receptionist breaks and lunches.
