About The Position
We are seeking a Social Worker to provide psychosocial support and services to residents in a continuing care retirement community under the supervision of the Director of Social Services. This is a part-time, temporary position in Delaware County.
Requirements
- BSW is required

- Experience with family systems
- Experience with the aging population
- Experience with discharge planning
- Prior work in a CCRC or long-term care setting
Responsibilities
- Provide psychosocial support and services to residents in a continuing care retirement community
- Assist with discharge planning
- Complete MDS documentation
- Address residents' psychosocial needs
- Collaborate with the interdisciplinary team
