The LPN will conduct daily nursing functions in accordance with current rules, regulations, and guidelines that govern the long-term care facility/assisted living communities. They will ensure that all nursing personnel assigned comply with written policies and procedures, including the safety program. The role involves admitting, transferring, and discharging residents with appropriate documentation, participating in resident and family orientation, and making rounds with healthcare providers and hospice/home health staff as needed. The LPN will utilize nursing personnel qualifications according to resident care plans, service plans, and healthcare provider orders. Key responsibilities include completing daily clinical and environmental rounds, resolving resident and family issues, participating in facility surveys, managing clinical programs (e.g., dementia, wound care, QA, infection control, admissions), completing initial assessments and developing interim care plans, updating and reviewing care plans, communicating with healthcare providers, reviewing resident medical records, arranging diagnostic and therapeutic services, communicating policies and procedures, providing shift reports, completing accident/incident reports, ensuring accurate medical record documentation, providing resident and family information for conferences, reporting discrepancies, and ensuring rooms are ready for new admissions. The LPN will also greet new admissions, notify practitioners and families of condition changes, and assist in arranging transportation and other third-party services. Medication administration includes preparing and administering medications, communicating with the pharmacy, ensuring adequate supplies, and reviewing medication administration records. Personnel functions involve supervising the nursing unit, working with the Director of Nursing to resolve staff performance issues, and reporting complaints and grievances. Safety and sanitation include assuring protective equipment, maintaining clean work areas, recommending equipment and supply needs, and ensuring adequate stock levels. The role also includes performing other job-related duties as assigned.
