About The Position
The Retreat at Covington is seeking a Wellness Nurse (LPN/Nurse MedTech) to join their team. This role reports directly to the Wellness Director and is responsible for monitoring the health and wellbeing of residents, providing leadership support, and assisting with resident care and services. Responsibilities include overseeing resident care associates, communicating with resident support services and families, maintaining clinical quality assurance, and leading wellness initiatives. The role also involves direct resident care after hours if needed.
Requirements
- Must be a Licensed Practical Nurse or Registered Nurse
- Demonstrated Leadership capabilities
- Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
Nice To Haves
- A minimum of one year of experience working with elders or disabled individuals
- Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing
Responsibilities
- Monitor the health and wellbeing of residents.
- Provide leadership support and assistance with resident care and services.
- Oversee resident care associates.
- Communicate with resident support services and families.
- Maintain clinical quality assurance.
- Lead, demonstrate, and support all elements of wellness.
- Provide direct care of residents after hours if needed.
- Identify on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process.
- Ensure proper follow through and documentation for residents with a change in clinical care needs.
- Assist the Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly.
- Inform the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
- Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
- Assist with direct resident care as needed.
