About The Position

We are currently seeking a talented, compassionate LPN to provide hospice care to patients where they reside. This position will be based out of Largo, MD and will cover both Prince George's County and Anne Arundel County. This position is a night shift, fulltime - 3 12-hr shifts per week, 8pm - 8:30am with a weekend rotation required. This position is Non-Exempt, and paid hourly + differential.

Requirements

  • Current Maryland LPN license and thorough knowledge of nursing principles.
  • 2 or more years of nursing experience in med/surg or home care required.
  • Familiarity and comfort with documenting in an Electronic Medical Record.
  • Valid driver’s license and acceptable driving record and reliable transportation
  • Ability to meet the physical demands of the position with or without reasonable accommodation.
  • Employees be vaccinated against COVID and influenza, as well as being screened for TB upon hire and annually thereafter.

Nice To Haves

  • experience in hospice, end-of-life, or critical care experience preferred.

Responsibilities

  • Engage with patients to build trust while you assess their physical, psychosocial, and emotional condition as well as their environment during regular nursing visits.
  • Adapts communication style to each individual and circumstance to provide an atmosphere that protects patient dignity.
  • Recognizes changes in clinical status, anticipates patient and caregiver needs with a sense of urgency, and reports any changes to the patient or their environment to the interdisciplinary team to ensure the highest level of service and care.
  • Requests assistance from supervisor or an RN as needed.
  • Work collaboratively with your supervising RN Case Manager to initiate and revise plan and goals of care to ensure patient comfort and family/caregiver support and competence throughout their journey through advanced illness and end-of-life.
  • Follows nursing standards of practice and meets or exceeds HOC quality goals to ensure the best possible outcomes for the patient and family from admission through bereavement care.
