About The Position

The Licensed Practical Nurse (LPN) at EHM Senior Solutions is responsible for providing high-quality nursing care to residents, ensuring their comfort and well-being. The role involves collaborating with healthcare teams to deliver personalized care plans and support the overall mission of enhancing the quality of life for seniors.

Requirements

• Completion of a state-approved Licensed Practical Nurse (LPN) program.
• Current and valid LPN license in the State of Michigan.
• Strong understanding of nursing care practices and procedures.
• Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.
• Ability to assess patient health needs and provide appropriate care and interventions.
• Proficient in maintaining accurate and detailed patient records.
• Ability to work independently and as part of a team in a fast-paced environment.
• Compassionate and patient-centered approach to care.
• Basic computer skills for documentation and communication purposes.

Nice To Haves

• Minimum of 1 year of experience working as a Licensed Practical Nurse, preferably in a senior care or assisted living environment.
• Experience in administering medications and monitoring patient responses.
• Experience in collaborating with healthcare teams to develop and implement patient care plans.

Responsibilities

• Administer prescribed medications and perform routine medical procedures under the supervision of a registered nurse or physician.
• Monitor residents' health by checking vital signs and observing changes in their condition, reporting any concerns to the healthcare team.
• Assist residents with daily living activities, including bathing, dressing, and mobility, to promote independence and dignity.
• Document and maintain accurate records of residents' health status, treatments, and progress according to facility protocols.
• Collaborate with other healthcare professionals to develop and implement individualized care plans tailored to residents' needs.
• Provide emotional support and education to residents and their families, addressing concerns and promoting a positive environment.
• Participate in ongoing training and professional development to stay current with nursing practices and enhance skills.
• Ensure compliance with all healthcare regulations, policies, and procedures to maintain a safe and effective care environment.
