About The Position

The LPN (Licensed Practical Nurse) will direct the day-to-day functions of the nursing assistants and meet with nursing staff to plan services, programs, and activities. This role involves comprehensive recordkeeping, including resident admissions, transfers, and discharges. The LPN will transcribe physician's orders, chart nurses' notes, and communicate new admissions to staff. They will also participate in facility surveys, orientation of new residents and families, and rounds with physicians. Collaboration with physicians on resident care and reviewing resident charts for specific treatments and medications are key aspects. The LPN is responsible for administering medications and treatments, ensuring adequate supplies, and performing various nursing services such as catheterization, wound care, and specimen collection. They will also monitor vital signs, check food brought by visitors, and provide health care instruction to residents and families, while maintaining strict confidentiality of resident information. The role requires providing service excellence in line with the organization's values of compassion, accountability, respect, excellence, and service.

Requirements

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN program.
  • Must possess a current, unencumbered, active LPN license to practice as an LPN in this state or per House Bill 4466 and Senate Bill 2708, be a Nursing Program Graduate which allows a graduate nurse to function at the bedside under appropriate supervision through May 21, 2025.
  • BLS / CPR and AED Program Certification is required.
  • Must be able to read, write, speak, and understand the English language.
  • Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of team work and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must be able to lift a minimum of 50 pounds and meet all other physical and sensory requirements of the job.

Responsibilities

  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
  • Meet with assigned nursing staff and support personnel to plan shifts’ services, programs, and activities.
  • Complete and file required recordkeeping forms/charts upon the resident’s admission, transfer, and/or discharge.
  • Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.
  • Receive telephone orders from physicians and record on the Physicians’ Order Form.
  • Transcribe physician’s orders to resident charts, cardex, medication cards, treatment/care plans, as required.
  • Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care.
  • Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc.
  • Participate in facility surveys (inspections) made by authorized government agencies as requested.
  • Perform nursing administrative functions as outlined in the job description.
  • Thoroughly chart and document by completing and filing required recordkeeping forms/charts upon the resident’s admission, transfer and or discharge.
  • Participate in the orientation of new residents/family members to the facility.
  • Make rounds with physicians as necessary.
  • Consult with the resident’s physician in providing the resident’s care, treatment, rehabilitation, etc., as necessary.
  • Review the resident’s chart for specific treatments, medication orders, diets, etc., as necessary.
  • Prepare and administer medications as ordered by the physician. Verify the identity of the resident before administering the medication/treatment.
  • Perform all tasks as an LPN pertaining to the Drug Administration tasks of the job, as outlined in the job description.
  • Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor.
  • Administer professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required.
  • Collects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples.
  • Take and record TPRs, blood pressures, etc., as necessary.
  • Check foods brought into the facility by the resident’s family/visitors to ensure that it is within the resident’s dietary allowances. Report problem areas to the Nurse Supervisor and Dietary Supervisor.
  • Provide health care instruction to patient and family members.
  • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information; and adhere to Resident’s rights.
  • Perform other work related duties as assigned or requested.
