About The Position

Wellpath is seeking an LPN to provide care to underserved individuals. This role is crucial in patient care, contributing to the nursing process of assessment, planning, implementation, and evaluation. The LPN will work under the supervision of RNs, providing direction and guidance in patient education and activities aligned with their competencies. Requirements

  • Graduate from an accredited school of nursing.
  • Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
  • Must be able to obtain and maintain CPR certification.

  • A minimum of one (1) year clinic experience.

Responsibilities

  • Assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning.
  • Consults with RNs and staff as needed.
  • Implements treatment programs directed by Health Care Practitioners.
  • Administers medications, including IV therapy (if certified).
  • Performs diagnostic tests according to established protocols and procedures.
  • Collaborates in controlled substance counts at the beginning of each shift.
  • Adheres to safety techniques, procedures, and standard precautions.
  • Responds to codes or health emergencies within standard guidelines.
  • Assists in medical or minor surgical procedures.
  • Implements nutrition and therapeutic diet plans.
  • Documents nursing encounters using SOAP charting.
  • Ensures effective communication with nursing staff, physicians, and other relevant personnel.
  • Attends mandatory staff meetings and training sessions.
  • Demonstrates a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care.
