About The Position

Provides quality resident care by performing assessments, formulating, implementing, and evaluating resident care plans. Maintains open communications with the interdisciplinary healthcare team, assures and maintains a safe and infection-free resident environment, acts as a role model and assumes a leadership role as designated, and continually upgrades and makes changes in the job area to assure professional growth and development. Supports, promotes, and projects guest relations and positive images to all visitors, residents, and other Waveny LifeCare Network personnel.

Responsibilities

  • Performs initial resident assessment within three days of admission and continues the assessment process on an ongoing basis using applicable skills.
  • Updates Resident Care Plans identifying resident needs and defining goals in terms of observable and measurable outcomes.
  • Implements resident care according to the medical/nursing care plans assuring sound, accurate, and comprehensive care is delivered to each resident.
  • Administers medications as ordered by the physician in accordance with Waveny Care Network policy, assuring residents receive the correct medications and doses while understanding all aspects of medications.
  • Follows up with healthcare providers, pharmacies, and residents' POAs to ensure adequate medication supplies are available and refilled in a timely manner for each monthly medication cycle.
  • Administers or assists with treatments as ordered or performed by the physician.
  • Has knowledge of residents' conditions at all times, reporting changes to the appropriate discipline and recognizing all physical and psychological conditions.
  • Acts as a charge nurse as assigned, assuming 8-hour responsibility to recognize and meet residents' medical/nursing care needs.
  • Acts as liaison between residents, family, and physician, providing continuity of care and recognizing the family's understanding of physician's orders.
  • Performs direct resident care as assigned.
  • Assures resident education of care plans, having residents understand good health habits, special procedures, understanding of diagnosis, medications, and discharge planning, if applicable.
  • Performs any duties within the staff nurse's ability as may be assigned by the Director or Supervisor.
  • Identifies, helps prevent, and responds to incidents of elder abuse, neglect, and exploitation, including financial exploitation, and reports concerns to the appropriate authorities in accordance with state regulations.
  • Documents on charts accurately, objectively, and timely, reflecting problems, approaches, measurable goals, and time frames of resident care, complying with the Electronic Medical Record system.
  • Communicates appropriate information (personnel problems and residents' conditions) to Nurse Supervisor/RN-SALSA/RN-Designee/ED, assuring the meeting of physical, social, psychological, and spiritual needs of the resident.
  • Recognizes safety hazards and acts upon them appropriately.
  • Assures equipment is in good working order and reports any unsafe equipment to Environmental Services immediately.
  • Remains knowledgeable in emergency situations and uses equipment, documents, and communicates appropriately.
  • Recognizes potential infection control problem areas and maintains accountability for adherence to infection control procedures.
  • Exercises good judgment in using proper body mechanics to prevent injury to self and others.
  • Investigates and documents incidents, accidents, and resident concerns. Follows up with medical providers, POA, and Supervisors promptly.
  • Maintains ongoing communication with families, POAs, and healthcare providers to provide updates on residents' health status, care needs, and significant changes in condition.
  • Monitors residents for behavioral changes, wandering, elopement risks, and safety concerns.
  • Sets an example by displaying positive qualities and attitudes.
  • Provides direction and support to CNAs and other licensed personnel in the implementation of resident care.
  • Institutes and maintains a cooperative team effort among staff members.
  • Assists in the training and teaching of CNAs' required skills.
  • Reorganizes assignments, as needed, to deal with unexpected changes without compromising resident care.
  • Functions effectively in the Nurse role as designated and defined by the Nurse responsibilities and standards.
  • Actively participates in continuing education programs as needed and attends departmental meetings and in-services and all Waveny LifeCare Network mandatory in-services.
  • Remains knowledgeable in methods, techniques, and skills required in the field, complying with the Nursing Department's policies and procedures.
  • Keeps abreast of changes in the field through reading journals, membership in professional organizations, and attendance at professional conferences.
  • Communicates changes in the job field to their coworkers.
  • Creates and maintains an atmosphere of warmth, personal interest, and a positive and calm environment throughout the facility.
