About The Position

The LPN works under the supervision of a Registered Nurse (RN) or physician, and acts as first-line supervisor to resident/medication aides. This position provides assistance in the oversight of residents’ overall activities of daily living. Primary responsibilities include monitoring and assisting residents with medications, alerting relevant individuals of changes in resident conditions, and following up with all resident care issues while maintaining clinical quality assurance in accordance with New York State regulations and adherence to Longview's risk management practices. This role also assists residents with daily personal care and supports resident engagement in social and recreational activities which promote their physical and emotional health. The successful candidate will be passionate about helping others and making a positive impact in the lives of seniors. Strong communication skills, patience, and a compassionate approach are essential as this position works closely with residents, their families, and the healthcare team to deliver high-quality care.

Requirements

  • Must be at least 18 years of age.
  • Hold a current, unrestricted LPN license in New York State and remain in good standing.
  • Maintain current CPR and First Aid certification.
  • One (1) year of relevant experience in a healthcare setting, or an equivalent combination of education, training, and experience.
  • Demonstrated leadership skills, including sound judgment, problem-solving, and decision-making abilities.
  • Medication Administration Training (MAT) certification preferred; willingness to obtain required.
  • Strong interpersonal skills with the ability to communicate clearly, compassionately, and effectively with residents, families, and team members.
  • Excellent written and verbal communication skills.
  • Knowledge of resident rights and commitment to resident-centered care.
  • Must complete and maintain required training and certifications, including Bloodborne Pathogens, within three (3) months of hire, if not already obtained.
  • Ability to lift, carry, push, and pull up to 50 pounds and safely move equipment and supplies as needed.
  • Ability to stand and walk for extended periods and frequently bend, stoop, reach, and perform other physical duties associated with resident care.
  • Flexibility to work varying schedules and respond to emergency staffing needs.
  • Successful completion of a post-offer, pre-employment physical examination required.

Responsibilities

  • Provide high-quality nursing care and support residents with activities of daily living in an Assisted Living setting.
  • Conduct resident rounds and monitor resident safety, well-being, and response to nurse call systems.
  • Observe, assess, and document changes in resident condition, reporting concerns to the appropriate healthcare providers.
  • Administer medications and treatments according to physician orders and facility policies.
  • Maintain accurate and timely documentation in the electronic medical record (EMR), including medication administration and resident progress notes.
  • Supervise and support Medication Technicians and Resident Aides to ensure quality care and regulatory compliance.
  • Communicate effectively with residents, families, physicians, pharmacies, and interdisciplinary team members.
  • Assist with admissions, care planning, incident reporting, and discharge follow-up as needed.
  • Conduct routine audits and reviews of medication records, resident charts, and clinical documentation.
  • Participate in staff meetings, training programs, and quality improvement initiatives.
  • Maintain current licensure and required certifications.
  • Perform other duties as assigned to support resident care.
