About The Position

The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER plays a crucial role in providing high-quality nursing care to patients within the behavioral health setting. The LPN is responsible for supporting the healthcare team in delivering patient-centered care, ensuring the safety and well-being of patients during their stay, and contributing to the overall therapeutic environment.

Requirements

Completion of an accredited Licensed Practical Nurse (LPN) program.
Current, active LPN license in the state of practice.
Minimum of 3 years of experience as a Licensed Practical Nurse, preferably in a behavioral health setting.
Experience working with diverse patient populations, including those with mental health and substance abuse issues.
Demonstrated experience in administering medications and providing patient care under the supervision of a registered nurse or physician.
Strong understanding of behavioral health principles and practices.
Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
Ability to maintain accurate and detailed patient records.
Proficiency in using electronic health records (EHR) systems.
Strong organizational skills and the ability to manage multiple tasks efficiently.
Compassionate and patient-centered approach to care.
Ability to work effectively in a team-oriented environment.
Basic life support (BLS) certification is required.

Responsibilities

Administer medications and treatments as prescribed by healthcare providers, ensuring adherence to protocols and safety standards.
Monitor and document patient vital signs, symptoms, and progress, reporting any changes to the healthcare team promptly.
Assist in the development and implementation of individualized care plans, collaborating with registered nurses and other healthcare professionals.
Provide direct patient care, including assistance with daily living activities, personal hygiene, and mobility, while maintaining patient dignity and respect.
Participate in patient assessments and contribute to the evaluation of care plans, ensuring they meet the needs of the patients.
Maintain accurate and timely documentation of patient care activities, ensuring compliance with regulatory and organizational standards.
Support a therapeutic environment by engaging patients in activities and promoting positive interactions.
Communicate effectively with patients, families, and the healthcare team, fostering a collaborative and supportive atmosphere.
Uphold the standards of practice and ethics for Licensed Practical Nurses, adhering to all policies and procedures of LIFESTREAM BEHAVIORAL CENTER.
Assist in crisis intervention and management, ensuring patient and staff safety during challenging situations.
