About The Position
St. Francis Hospice (SFH) is growing and needs a Licensed Practical Nurse (LPN) to join their team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout the community. This role involves making a difference in the community by providing meaningful work that impacts the lives of patients and families daily.
Requirements
- Current and active Florida (LPN) License required.
Nice To Haves
- Hospice, home health, palliative care, or related experience preferred.
Responsibilities
- Accurate assessment of each patient’s status, problems and needs.
- Collection and analysis of data from the patient’s medical history, nursing assessments and staff input on an ongoing basis.
- Providing PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD.
- Providing care for patients for the entire County.
- Working collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.
