About The Position

DDRC is seeking a Licensed Practical Nurse (LPN) to provide nursing care and coordination for individuals in community-based settings. This role supports person-centered care, works closely with interdisciplinary teams, and ensures high-quality, compliant healthcare services outside of a traditional hospital environment.

Requirements

  • Active LPN license in Colorado (or ability to obtain)
  • Knowledge of medication administration and nursing standards
  • Experience in developmental disabilities, home health, or similar settings preferred
  • Strong communication and documentation skills
  • Ability to work independently and with a team

Responsibilities

  • Deliver nursing care, including assessments, medication coordination, and physician order implementation
  • Coordinate care and communicate with providers, teams, and families
  • Maintain accurate, compliant medical records
  • Act as liaison with physicians, pharmacies, and other providers
  • Monitor health changes and coordinate follow-up care
  • Ensure timely completion of exams, consents, and orders
  • Train and guide staff on medical procedures and infection control
  • Support quality reviews and regulatory compliance
  • Participate in on-call rotation
  • Mentor and onboard new nursing staff
  • Provide specialized support (e.g., G-tube care)
  • Lead or assist with trainings, committees, and projects
