About The Position

The Flex Team LPN provides clinical services on an as-needed basis across Christian Living Communities (CLC) communities, including skilled nursing and assisted living. The role coordinates care across different shifts and tailors services to residents’ individual needs and preferences, providing holistic, person-centered care. Team members are expected to work at least two shifts per month, most of which are 12 hours. This PRN position offers flexibility to select multiple locations, dates, and shifts in coordination with the Community Staffing Team.

Requirements

Colorado State Licensed Practical Nurse license in good standing.
Ability to work effectively with different teams in different communities.
Computer word-processing and database experience.
Ability to read, write, and speak English.
Excellent customer service skills.

Nice To Haves

One year of experience in a long-term care facility and coordinating care in a clinical environment.
Experience caring for residents with dementia.
IV certification.
One year of experience using electronic medical record platforms.

Responsibilities

Participate in resident care conferences and maintain resident care plans.
Identify signs and symptoms and take appropriate clinical action using sound judgment.
Communicate resident status to nursing staff, physicians, pharmacists, families, and other relevant parties.
Maintain resident privacy and confidentiality.
Complete electronic documentation.
Prepare, administer, and document medications, and monitor medications for expiration.
Communicate resident concerns, incidents, and progress to relevant parties.
Document assessments, interventions, and follow-up care.
Oversee and coordinate services during the shift, reporting issues to the Director of Nursing or supervisor.
Complete required onboarding training and periodic updates.
Perform other duties as assigned.
