About The Position

Horizon Health is seeking a Licensed Practical Nurse (LPN) for Foundations Health, its opioid treatment clinic in High Point, North Carolina. The OTP LPN delegates, coordinates, and evaluates patient care in collaboration with interdisciplinary team members and in accordance with the nursing process and OTP standards of care and practices.

The schedule is Monday through Friday, 5:00 a.m. to 2:00 p.m. Horizon Health describes opportunities for career growth and promotion from within. Its programs receive dedicated marketing support and resources from a national psychiatric contract management company.

Requirements

• Current North Carolina LPN license.

Nice To Haves

• Bachelor's degree preferred.
• At least two years of experience in an addiction care setting, such as inpatient, detox, partial hospitalization, or OTP, preferred.

Responsibilities

• Dispense methadone and other medications according to physician orders and clinic policy.
• Accurately record patient information in patient records.
• Briefly assess patients' mental and physical well-being and monitor for signs and symptoms associated with licit and illicit drug use.
• Participate in urine drug screening (UDS) surveillance according to policy.
• Coordinate medication reviews and staffing with agency physicians.
• Complete required controlled-substance inventories to ensure accurate medication counts.
• Conduct patient education programs based on identified needs, including health and wellness education.
• Participate in treatment team meetings and provide feedback to the interdisciplinary team.
