This position involves conducting daily nursing functions in accordance with current rules, regulations, and guidelines governing long-term care facilities and assisted living communities. The role requires ensuring nursing personnel comply with policies and procedures, managing resident admissions, transfers, and discharges, and participating in resident and family orientation. The LPN will collaborate with healthcare providers, hospice, and home health staff, and utilize nursing personnel qualifications according to resident care plans. Key responsibilities include daily clinical and environmental rounds, resolving resident and family issues, participating in facility surveys, and potentially managing clinical programs such as dementia care, wound care, quality assurance, infection control, and admissions. The role also involves completing initial assessments, developing interim care plans, updating and reviewing care plans based on nursing needs and provider orders, and assisting with service plans. Effective communication with healthcare providers, pharmacy, and nursing management is crucial, along with accurate and timely documentation, including accident/incident reports and progress notes. The LPN will also be responsible for medication administration, ensuring adequate supplies, and supervising nursing units to ensure adherence to nursing standards. Safety and sanitation are paramount, requiring the maintenance of clean and organized work areas and resident rooms, and ensuring adequate stock levels of medications and supplies.
