About The Position

LPN – Licensed Practical Nurse (Home Health) Stamford/Norwalk | Full-Time, Part-Time, or Per Diem Opportunities Available. Compensation: This is a paid per visit position with routine visit rates starting at $35.00 per visit! We’re looking for caring and dedicated Licensed Practical Nurses (LPNs) to join our growing home health team.

Requirements

• Active LPN license in CT
• Physical exam within the past 12 months
• Current TB test (within 12 months) and CPR certification
• Valid driver’s license
• Ability to attend a 2-day orientation at our Stamford office

Responsibilities

• Obtain medical history and assess patient conditions
• Conduct physical assessments including vitals and mental status
• Provide skilled nursing care per physician’s plan of care
• Maintain patient confidentiality in compliance with HIPAA
• Evaluate patient progress and communicate updates to the team
• Submit all visit notes and documentation promptly
• Collaborate with RNs and other care team members
• Educate patients and families on care plans and home management
