About The Position
Exciting Opportunity for the Licensed Practical nurse (LPN) to be part of a committed and compassionate care team. URMC has the privilege of opening a 40 bed, newly renovated long term care unit at St. Johns Nursing home. This unit will accept alternate level of care (ALC) patients from hospitals within the URMC system. This is an exciting opportunity to partner with the team at St. Johns to open and build a sustainable model to care for the patients admitted to this unit. The candidates must demonstrate strong critical thinking skills, flexibility, communication, a collaborative spirit, and a willingness to be part of a trailblazing team.

Nice To Haves
- Experience in Long term care preferred but not required
Responsibilities
- In an inpatient setting, remains accountable for provision of direct care to assigned patients.
- Contributes to unit activities and supports enhancement of the care delivery system.
- Works as a team member and assists other staff members with implementing patient care.
- Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD.
- Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication.
