About The Position

At MAB Community Services we are dedicated to providing compassionate and specialized care to individuals with acquired brain injuries in our residential group home environment. Our mission is to empower our residents to achieve their highest potential while ensuring their health and well-being. Position: MAB is looking for a caring, compassionate & flexible LPN to provide part time nursing wound care to a group home resident. Great flex hours are available for retired nurses, LPNs in school, nurses looking for mother's hours or looking for a change of pace from the fast hospital scene or Rehab facilities. Ideal for LPNs who live in or are familiar with the Watertown area. Looking to fill 3 days of wound care a week. 4-hour shifts available. Requirements

  • Current state licensure as a Licensed Practical Nurse (LPN).
  • Previous experience in a healthcare setting, preferably with individuals with brain injuries or other neurological conditions.
  • Strong communication and interpersonal skills.
  • Ability to work effectively as part of a team and independently.
  • Compassionate, patient, and dedicated to providing high-quality care

Responsibilities

  • Provide direct nursing care to residents, including administering medications/injections, providing wound care, monitoring vital signs, and assisting with daily living activities.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Support residents in their rehabilitation goals and promote independence through personal care and therapeutic activities.
  • Maintain accurate medical records and documentation in accordance with regulations and standards.
  • Educate residents and staff on health management and care techniques.
  • Ensure a safe and clean living environment for all residents.
  • Participate in team meetings and contribute to the overall improvement of care practices.
  • Oversees MAP Program at the residence by conducting routine audits of documentation system, providing specific and systemic response to medication errors.
  • Conducts reviews of health care documentation to ensure program compliance with the provider’s recommendations as well as MAB, DDS policies and the MAP policies.
  • Develops and conducts in-service training to Adult Disability Service employees.
  • Identifies and facilitates the utilization of appropriate community resources.
  • Provides client-specific consultation to program staff, in conjunction with health care providers.
  • Transports participants to any medical appointments and provides advocacy at the appointment. Completes all follow up from the appointment.
