Requirements

  • Oregon LPN License
  • AHA BLS within 30 days
  • LPN Program
  • Two (2) years LPN experience

Nice To Haves

  • Medical, Surgical, Home Health or Wound Care experience

Responsibilities

  • Assesses the patient’s condition and nursing care needs, and reports complex care needs and/or changes in condition to the RN or Wound Care Provider when applicable.
  • Provides direct care in accordance with the prescribed medical and nursing treatment plans under the instruction of the Primary RN, Wound Care provider.
  • Implements and evaluates appropriate patient and family education per Plan of Care and under RN supervision.
  • Functions as an advocate for the patient and his/her family by supplying and interpreting information, supporting decisions, and facilitating communication.
  • Completes all visits and documentation within regulatory and Agency timeframes.
