About The Position

Assists with coordinating treatment to provide comprehensive wound assessments. Follows the plan of care and provides scheduled wound treatments as ordered by the physician. Assists with patient, family, and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections, and other therapies. Assists with the performance improvement plan of the organization.

Requirements

Graduate of a state-approved school of practical nursing and has passed the state board examination.
Current state License Practical Nurse Licensure.
Minimum of two (2) years’ experience in long term care nursing.
Current BCLS Certification.
Ability to communicate in English, both verbally and in writing.
Basic Computer Skills.
Knowledgeable of the anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems.
Follows Code of Conduct policy.
Adheres to dress code; appearance is neat and clean.
Completes annual educational requirements.
Maintains regulatory requirements.
Maintains patient confidentiality at all times.
Reports to work on time and as scheduled; completes work within designated time.
Wears identification when on duty; uses computerized time clock system correctly.
Completes in-services and returns in a timely fashion.
Attends annual review and/or skills fair and department in-services, as scheduled.
Complies with all organizational policies regarding ethical business practices.
Communicates the mission statement of the organization.
Always treat others in a friendly, helpful manner.
Refers co-workers to proper sources when unable to provide an answer.
Interacts with others in a professional and friendly manner.

Responsibilities

Perform wound assessment, reassessment, and management of all patients per policy, including geriatric and general populations.
Utilize principles of current wound management theory when providing care to patients with acute and chronic wounds.
Demonstrate thorough knowledge of staging systems of wounds.
Follow skin care programs to prevent skin breakdown.
Follow plan of care.
Adequately assess and reassess pain.
Perform treatments and provide services to the level of licensure.
Assist with a teaching plan based upon identified learning needs and evaluate the effectiveness of learning, including family as appropriate.
Knowledgeable of medications and correct administration based on patient age and clinical condition, following the five medication rights.
Identify and address psychosocial needs of patients and families; communicate with social service/discharge planner as appropriate.
Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Assist with in-services on wound prevention, wound care, ostomy care, and incontinence for nursing staff.
Communicate appropriately and clearly to nurse managers, nursing staff, and physicians.
Consult other healthcare professionals/departments as appropriate to provide an interdisciplinary approach to patient needs.
Ensure documentation meets current standards and practices.
Stay current with changes, new advances, and treatments in wound care.
